Informed voluntary consent of the client for psychological counseling is a document that, on the one hand, seems formal, but, on the other hand, it regulates all the main organizational issues between the client and the psychologist. In this article I decided to publish my consent format, I hope that it will be very useful for many. INFORMED VOLUNTARY CONSENT of the client to individual psychological counseling I, __________________________________________________________________________, give voluntary consent to individual psychological counseling, and am also familiar with the following conditions for the provision of psychological counseling services.I. Privacy1. The psychologist has no right to disclose the content of consultations, as well as the identity of the client, to anyone and under any circumstances, except in cases that threaten the life and health of the client and other persons.2. Disclosure of confidentiality in order to protect the life and health of the client and other people is prescribed by the laws of the Russian Federation.3. Everything discussed during the sessions, as well as the notes kept by the psychologist, are strictly confidential.II. Features of work4. The first consultation is for informational purposes, it is free and lasts 30 minutes (this is my personal point)5. All online work is carried out on the __________ platform.6. During the session, hidden video and audio recordings are not carried out.7. A psychologist has the right to privacy. If the client wishes to record any part of the session and/or post it on Internet sites, written permission must be obtained from the psychologist.8. During the work, the psychologist keeps brief notes that describe the topics of the session and comments. All records are stored in a secure place without the right of access to third parties.9. The client has the right to receive these records within 24 hours of the consultation. III. Conditions for disclosing the case10. A psychologist has the right to discuss difficulties when working with a client in the professional circle of psychologists, without revealing the client’s identity.11. If a psychologist wishes to publish a description of work with a client after completion of the counseling process, he will additionally contact the client to obtain written consent. IV. No dual relationships12. According to the Code of Ethics, the psychologist is not interested in the client's personality, his life or his environment outside of psychological counseling.13. The psychologist does not study the client’s profiles on social networks or professional communities.14. A psychologist has the right to refuse to work with a client if he is his relative, friend or colleague.15. The psychologist does not provide the client with services other than psychological ones and does not receive them from the client, does not enter into intentional communication with the client outside of counseling and for a long time after completion. V. Rules of communication16. Agreeing or canceling a consultation is carried out at ________________.17. If the psychologist does not respond to the client's message within 24 hours, the client should agree or cancel the consultation during a telephone call.18. Frequency of consultations – once a week.19. The client cannot call/write to the psychologist on issues not related to organizational issues. VI. Ending the session20. The client has the right to complete the consultation unilaterally as soon as he sees fit.21. The psychologist reserves the right to end the consultation unilaterally in several cases: 1) if during the consultation it turns out that the psychologist and the client have close friends in common; 2) if the psychologist is unable to help the client due to lack of skills and training; 3) if the client uses violence and threats against the psychologist, his family and environment. VII. Submitting claims22. If the client believes that the psychologist behaved inappropriately towards the client or violated ethics, it is necessary to discuss the complaint with the psychologist.23. The client has the right to publicly express claims, discuss the progress. ____________/______________
